I had occasion to test some of this for my work. The issues have more to do with the ePub and Kindle eReaders than Scrivener. Modern Kindles usually support about 1000 Glyphs of the Latin-1 font set. <$n> is the placeholder for chapter numbers like 1,2,3 usually used for top level folder.

<$hn> is the heiarchal placholder. It is usually used to number all binder entires like 1.2.3, 1.2.4 etc. by how they are nested in the binder.

To use sub-chapters I see two options. If your document is like a novel and has multiple scenes per chapter then you could play with the template for a "novel with parts". Treat each part as a chapter and the next level down as sub-chapters. You might be better off using something like the...

Windows also has text to speach. But as I remember you have to enable it and assign the keyboard shortcut.
