If you haven't already, I'd recommend taking a look at the Tutorial project, in which all is explained.

Think of the index card as being paper-clipped to your actual document. The card contains the title and synopsis, but not the document text.

Yes. The transition should be a non-event for iOS Scrivener.

Make sure the project is fully synchronized before you upgrade, just in case.
