I finally figured it out by accident. I'll post how to do it in case anybody else wants to know.

Go to NAVIGATE -> BINDER SELECTION AFFECTS -> LEFT EDITOR

BUG - Speech does not ignore Annotations

When I turn on speech, it does not ignore inline annotations. Instead, it just reads through them like regular text.

IMO, this is a bug. (At the very least, it ought to be selectable behavior, maybe a checkbox or something.)
