My line of thinking then would be that it should be using a “heading-flourish” class instead of something generic. It might very well be you’d want that for other reasons as well, to modify the spacing of the box a bit to design the element into the heading and away from the body text for example. I...

The Windows beta that is currently available may have some of this resolved already. The new version will indeed finally have a “unified” editor in Scrivenings mode, and that will mean an improvement to functions like this designed to work on all visible text.

You have to go through your text and mark the entries you want to show in the index.
